If you would like to request a copy of an Incident Report or other information, you must download the "Request for Information" form.
After you have printed the form and filled it out, you may submit it by one of the following ways:
- Fax it to (909) 621-5261
- Mail it to the Montclair Fire Department, P.O. Box 2308, Montclair, CA 91763(Attention: Records)
- Or stop by our office: 8901 Monte Vista Avenue, Montclair, CA 91763
Incident Reports are $1 per report including tax, effective August 16, 2013, per Resolution No. 13-2994.
